Acknowledgment of Risks:

I, the undersigned, acknowledge that using the indoor golf simulator involves inherent risks, including but not limited to, the risk of personal injury, property damage, or loss. I understand and accept that these risks are part of the activity.

Release of Liability:

In consideration of being permitted to use the indoor golf simulator, I agree to release, waive, discharge, and hold harmless JD's Golf Garage, its owners, agents, employees, and volunteers from any and all liability, claims, demands, or causes of action that may arise from my use of the facility, including but not limited to any injuries, damages, or losses.

Assumption of Responsibility:

I accept full responsibility for any injuries or damages that may occur to myself or others due to my actions or negligence while using the indoor golf simulator. I also agree to reimburse JD's Golf Garage for any damages to the equipment or property caused by my use.

Medical Condition:

I declare that I am in good health and do not have any condition that would prevent me from safely using the indoor golf simulator. I agree to stop using the simulator immediately if I experience any discomfort or health issues.

Agreement to Rules and Instructions:

I agree to follow all posted rules and instructions provided by JD's Golf Garage staff. I understand that failure to do so may result in the termination of my use of the facility without a refund.

No Outside Alcohol:

I acknowledge that no outside alcohol is permitted on the premises. I understand that any violation of this policy may result in immediate removal from the facility without a refund.

Photography and Video Release:

I consent to the use of any photographs or video recordings taken during my use of the indoor golf simulator for promotional purposes by JD's Golf Garage.

Severability:

If any part of this waiver is found to be invalid or unenforceable, the remaining provisions will remain in full force and effect.

Governing Law:

This waiver shall be governed by and construed in accordance with the laws of Colorado.

Acknowledgment of Understanding:

I have read and fully understand this waiver and release of liability. I am aware that by signing this document, I am waiving certain legal rights, including the right to sue JD's Golf Garage.

By signing this waiver, you acknowledge that you have read, understood, and agree to the terms and conditions outlined above.
